THE PROGRAM
A comprehensive home health care program is established including a plan of care with the patient's physician.
Daily nursing/aide notes are maintained, and our Registered Nurse conducts free on-site supervisory visits.
A complete patient file is maintained and frequently reviewed by a Registered Nurse.
Nurses as Needed's goal is to provide all necessary services on a professional level that encourage the patient's independence and security while living at home.
If the patient is in a health care facility, our goal is to provide maximum support and comfort.
In addition, we are committed to each patient's recovery at the earliest and most practical time frame while ensuring safe patient care.

COMPREHENSIVE CASE MANAGEMENT
- Initial home care evaluation/assessment by Registered Nurse.
- Skill matching to select most appropriate caregiver for case; appropriate scheduling of caregivers.
- Conference with physician to develop plan of treatment, and document orders, diagnosis, medications, etc.
- Supervisory visits to home to evaluate caregiver and assess client condition arranged in accordance with client approval and direction.
- Additional supervisory visits, as needed, for problem prevention/solving.
- As needed consultation with physician to adjust plan of treatment, medications, etc.
- Regular reports to/consultation with insurance representative on a schedule determined by insurance company.
- Coordination of ancillary services deemed necessary.
- Maintenance of detailed records on all aspects of care provided.
- Registered nurse supervisor on call 24 hours/day, 7 days/week.
- Maintenance of quality service standards, and ethics which are more stringent than state law or federal Medicare standards.
- Coordination of needed community resources.

- Phone is answered 24 hours/day, 7 days/week by a trained employee
- Caregivers are trained, supervised, and monitored by a Registered Nurse.

- Holistic management/assessment of personal needs as well as medical needs
- Direct contact with RN assigned to case
- Individual care plan created for each client.
